Becoming a Patient

We remove traditional barriers to accessibility, and provide a patient-centered approach to healthcare by empowering our patients with the knowledge and tools they need to achieve their desired health outcomes.

What We Provide

  • Ongoing Care

    Ongoing care is provided for patients with chronic conditions such diabetes, hypertension, high cholesterol, etc. This includes medication monitoring, educating patients on nutrition, exercise, and other healthy lifestyle habits.

  • Specialists & Diagnostics

    AWC is fortunate to have specialists who can treat a wide-range of medical conditions. However, referrals to outside specialists and diagnostics are offered when the patient requires more than what we can provide.

  • Holistic Health

    AWC believes in treating the whole patient and has adopted a holistic approach to healthcare. Mental health care and stress management services are provided to patients who need this type of healthcare support.

Prospective Patients

Many patients who visit AWC for the first time have not seen a healthcare provider in several years. During their initial visit, they have a comprehensive appointment with a nurse practitioner or doctor, including a complete lab workup. This first appointment, scheduled based on the patient's individual needs, is crucial for establishing trust. The provider takes the time to get to know the patient as a person, conducts a thorough physical examination, prescribes medication if necessary, and refers the patient to our other internal clinics or specialists for further testing or treatment.

AWC Qualification Guidelines

 
 

To qualify as an AWC patient :

  • You must not have any healthcare insurance, such as Medicare/Medicaid, Veterans Benefits Administration, or PeachCare

  • Having medical insurance, but not dental coverage, disqualifies you from dental services

  • Your household income (based on family size) must fall at or below 200% of the Federal Poverty Level (FPL) guidelines

  • If you claim “no income,” you must provide proof of household income determined by the previous year’s federal tax return *

    *Please bring acceptable proof of income from the Department of Labor or a recent Tax Return.

As part of our mission to serve the community, AWC is bound to certain Federal requirements for accepting patients. The 2024 Federal Poverty Level Household Income guidelines — based on family size — are as follows:

Family Size — 200% of FPL Household Income Guidelines

1 family member — $30,120
2 family members — $40,880
3 family members — $51,640
4 family members — $62,400
5 family members — $73,160
6 family members — $83,920
